Citizens have the right to fill out an application directly on the territory of the traffic police, having received the form from the employees. The application contains information:
Document header.
Applicant's full name, date and place of birth.
Registration address (temporary stay).
Passport details.
Message of the reason for replacing the document.
Series, number and Phone Number List date of issue of the expired document.
Date of completion, signature.
